Advanced Care Planning

Everyone should have a “living will.”  This ensures the patient that their wishes are followed explicitly.   A medical proxy or power of attorney is designated by you. Your medical proxy’s responsibility is to be able to speak on your behalf if you cannot make your wishes known on your own. Our clinicians guide you through this process and explain DNR, DNI, DNH, intubation, peg tube, tracheostomy, pain control, etc. An official will be generated that you can share with your primary doctor and family members
